The occipital transtentorial approach (OTA), which is often applied for superior cerebellar lesions, has an inevitable risk of homonymous hemianopsia due to the retraction lobe. endoscopic provides increased visibility surgical field wide-angled panoramic view and minimally invasive in approaching deep brain lesions compared conventional microscopic approach. However, little known regarding OTA removal lesions. We experienced a case hemangioblastoma paramedian surface cerebellum that was successfully treated with combined gravity while avoiding postoperative visual dysfunction.
